Portsmouth’s Adults Intermediate Care Team is seeking a dedicated and experienced Qualified Adults Social Worker to join our dynamic community team. This is a fantastic opportunity to make a real difference in the lives of vulnerable adults in our community, helping them to regain independence and live fulfilling lives.

Key Responsibilities:

• Conduct Care Act assessments and first reviews for adults across the intermediate care pathway.
• Work collaboratively across various settings including inpatient units, the acute hospital, and within the community (people's own homes, nursing homes, and residential homes).
• Participate in the team’s rotational duties, including working in our rehabilitation units during periods of high demand, particularly around discharge planning.
• Deliver high-quality, person-centered care that upholds the dignity and rights of service users.
• Work closely with multidisciplinary teams to ensure seamless and effective care delivery.

Requirements:

• Qualified Social Worker with a valid registration with Social Work England.
• Strong understanding of the Care Act and experience in undertaking comprehensive assessments and reviews.
• Ability to work flexibly across different care settings, adapting to the needs of the service.
• Excellent communication skills, with a commitment to collaborative working and supporting positive outcomes for service users.
